How to Choose an ACO

Finding success in value-based care starts with finding the right accountable care organization partner for you and your staff.

Aledade's track record of success.

Good ACOs can deliver savings once, but our member practices, clinics and health centers do it consistently and improve year after year.

Aledade’s platform has been instrumental in helping me maintain my independence. I have been able to continue fee-for-service while simultaneously improving my quality of care and being rewarded for that improvement.

How does joining an ACO work?
Our implementation timeline is designed to help you onboard quickly and seamlessly so you can start accessing the benefits of Aledade membership right away. This is the timeline of joining with an Aledade ACO.
You’ll receive a welcome email with an onboarding guide and a technical survey to help us assess your needs and make an implementation plan that aligns with your current systems.
We’ll have a discovery call to introduce your onboarding support team to your designated Practice Champion and review your technology survey and data sources.
We’ll lead a kickoff meeting to deliver an overview of Aledade, your ACO initiatives and a technology demo to your staff.
Our technology experts will help you optimize your EHR templates for ACO initiatives.
Training and integration will be underway, and you’ll begin diagnosis documentation training and clinical calls with your designated Aledade medical director.
This timeline may change depending on the individual primary care organization size and complexity.
You need a great ACO
While many ACOs may offer similar services and products, great ACOs set the standard for sustainable financial success, enhanced quality performance in patient care, and support tailored to the unique needs of your organization.
