Who needs a DA form 5888?
Active duty soldiers in the US army have the opportunity to participate in the Exceptional Family Member Program. This program provides medical and educational benefits for the exceptional family members (FM) of the soldier. An FM is any family member of an active duty soldier, regardless of age, with any physical, emotional, developmental or educational need that requires special therapy, treatment, training, education or counselling on an ongoing basis.
What is the DA form 5888 for?
The purpose of this form is to validate family members for deployment screening and provide authorities with the necessary data for making assignment decisions. If the screening is successful, EMP officials will find a facility where the special needs of the family member can be accommodated.
Is it accompanied by other forms?
If the screener indicates that EMP enrollment may be warranted, copies of applicable DA form 2792 (EMP Medical Summary) and/or DA 2792/1 (EMP Special Education/Early Intervention Summary) should be included in the completed nomination package.
When is DA form 5888 due?
This form expires in a year from authentication date given in Part B, block 11e. If the assignment selection process or training extends beyond one year, a new DA form 5888 and screening are required.
How do I fill out a DA form 5888?
This process should not take long. The form must be filled out after the screening. There is only one sheet to fill out with general information about an exceptional family member and the screening results.
